Membership connects you to the Community Choices family cooperative: a support, information, and resource network.
Community Choices provides opportunities throughout the year for our members to connect.
Our Connect department organizes family parties, social events, support groups, and advocacy training for members to fully engage with the cooperative and the greater community.
Our cooperative believes in the power of community, interdependence, and teamwork. Together we all create a stronger, more inclusive Champaign County!
Membership Eligibility Criteria
You must have a diagnosis of an intellectual or developmental disability
You must be on the PUNS list or have Medicaid waiver funding (HBS/CILA)
You must be 18 years old or older
You must live in Champaign County
Community Choices must be able to meet your needs
Our ability to meet your needs is based on:
- Proper documentation
- Talking with you and your family
- Our understanding of your needs
- Our ability to provide the support you require
How to become a Member:
Step 1
Complete our Contact Form.
Our Membership Coordinator will reach out to ask questions about your goals and tell you about our services. She will also determine your eligibility for certain programs.
Step 2
Schedule an intake meeting.
At the intake, you will show necessary documents to begin receiving services.
Step 3
Take part in our cooperative!
You are a member now! Attend events and meetings to get to know other folks and their families!
